One packet must be completed for each livestock entry. Date Due: Large animals - April 1; Small animals - May 27 Fee: None Amador County Bred & Raised Birth Certification Form Declaration that beef, sheep, goat, swine or rabbit was bred and raised within Amador County. Date Due: Must be submitted with Livestock Entry Form Declaration of Medication Form Declaration of medication used/not used for beef, sheep, goat, swine, poultry or rabbit. Use separate form for each animal. Date Due: July 26 upon check-in Adult and Junior Livestock Entry Form For all livestock entrants, including 4H, FFA, Jr. Grange and independants.
