All participants under the age of 18, or otherwise noted
in Entry Form, must have

All junior exhibitors must complete the entry forms,

record-keeping forms inclosed in the packet. One packet must be
completed for each livestock entry.

County Bred & Raised Birth Certification Form
Declaration that beef, sheep, goat, swine or rabbit
was bred and
raised within Amador County.
Date Due: Must be submitted with Livestock Entry Form

Declaration of medication used/not used for beef, sheep, goat,
swine, poultry or rabbit. Use separate form for each animal.
